HSA vs PPO Calculator

Open enrollment comes down to one question: which plan costs less for the care you will actually use? Enter both plans from your benefits packet to see the net cost of each, counting your employer's HSA money and the full tax saving, and the level of care at which they cross.

The cheaper plan in each row is in green. A negative cost means the employer's HSA money and the tax saved are more than you pay that year. The worst year for the HDHP is its out-of-pocket maximum of $6,000; keep at least that much in the HSA or savings.

What the HSA keeps doing after this year

Money you do not spend stays in the HSA, can be invested, and comes out tax-free for medical costs at any age; after 65 it can be withdrawn for anything and taxed like an IRA. This comparison counts only 2026, so it understates the HSA for anyone who can leave the money invested.

How this is worked out. Net cost is your premium plus what you pay for care, less the employer's HSA money and the tax your HSA contribution saves. With the FSA, the PPO's cost includes any election you do not spend. Spending is the plan's allowed amount for the care, before it pays.

2027 limits: HSA $4,500 self-only and $9,000 family, plus $1,000 at 55; HDHP deductible at least $1,750 / $3,500 and out-of-pocket at most $8,700 / $17,400. A general-purpose health FSA, yours or a spouse's, makes you ineligible for an HSA. Copays, separate drug tiers, out-of-network care and family embedded deductibles are not modelled; use each plan's summary of benefits and coverage for the figures.

Compare net cost, not premiums

The PPO's higher premium buys a smaller bill when you use care. The fair figure is premiums plus out-of-pocket, less the employer's HSA money and the tax you save. That net cost changes with how much care you use, so the calculator draws it for every level from nothing to a bad year.

The tax saving is bigger than your bracket

HSA contributions through payroll skip federal income tax, most states' tax and the 7.65% FICA tax. In the 22% bracket with a 5% state rate, every dollar costs you about 65 cents. Most comparison tools count only the federal bracket.

Plan for the bad year

The HDHP's worst case is its out-of-pocket maximumplus premiums. If a large bill would be hard to pay, keep at least that much in the HSA or in savings, or weigh the PPO's lower maximum more heavily than the averages suggest.

Give the PPO its FSA

A PPO can be paired with a health FSA, which also shelters out-of-pocket costs from tax. It is fairer to compare with the FSA switched on, as long as you would really spend the election; unspent FSA money is generally lost.

Frequently asked questions

Is an HDHP with an HSA cheaper than a PPO?

Often, but it depends on how much care you use. The HDHP usually has lower premiums, an employer HSA contribution and a tax deduction for what you put in, while the PPO pays more of each bill. For light and moderate users the HDHP tends to win; in a heavy year the answer turns on the two out-of-pocket maximums and the premium gap. The calculator finds the spending level at which the plans cost the same.

What are the HSA contribution limits for 2027 and 2026?

For 2027, $4,500 for self-only coverage and $9,000 for family coverage; for 2026, $4,400 and $8,750. Add $1,000 at 55 or older, and each spouse needs their own HSA for their catch-up. The limit includes your employer's contributions. To contribute at all, your plan must be a qualifying high-deductible plan: for 2027 a deductible of at least $1,750 self-only or $3,500 family and an out-of-pocket maximum of no more than $8,700 or $17,400 (for 2026, $1,700 / $3,400 and $8,500 / $17,000).

Can I open an HSA with an ACA marketplace bronze plan?

Yes, from 2026. The 2025 tax law treats bronze and catastrophic plans bought as individual coverage through a marketplace as high-deductible plans, even if their deductible or out-of-pocket maximum would not otherwise qualify. Identical plans bought off the exchange also count; small-business (SHOP) bronze plans do not. The same law made telehealth before the deductible permanently allowed and lets you pair an HSA with a direct primary care arrangement of up to $150 a month.

What stops me from contributing to an HSA?

You must be covered by a qualifying high-deductible plan and have no other disqualifying coverage. The common traps are a general-purpose health FSA, including one through a spouse's employer, enrolling in Medicare (the limit drops to zero from the first month of entitlement), and being claimed as someone's dependent. A limited-purpose FSA for dental and vision is fine.

How much tax does an HSA save?

Contributions through payroll avoid federal income tax, most states' income tax and the 7.65% FICA tax. At a 22% federal bracket and a 5% state rate, $3,400 of payroll contributions saves about $1,178. Contributions made directly, outside payroll, are deductible on your return but do not get the FICA saving. Above the Social Security wage base ($184,500 in 2026, $192,300 in 2027), only Medicare's 1.45% is saved.

Do California and New Jersey tax HSA contributions?

Yes. Both states tax HSA contributions and the account's earnings each year, so residents get only the federal and FICA savings. A California bill to conform for 2026–2030 died in January 2026. The calculator drops the state saving when you tick the box.

Should I use an FSA with a PPO?

If you know you will have out-of-pocket costs, a health FSA lets you pay them with pre-tax money, which narrows the gap with the HSA. The catch is that unspent FSA money is generally lost at the end of the plan year, apart from a limited carryover or grace period if your employer offers one. The 2026 FSA limit is $3,400; the 2027 figure had not been published when this was written.

What happens to HSA money I do not spend?

It stays yours, even if you change jobs or plans. It can be invested, grows tax-free, and comes out tax-free for qualified medical costs at any age. After 65 you can also withdraw it for anything, paying income tax like an IRA withdrawal. This calculator compares one year only, so it leaves that long-term value out.

What if I contribute too much to my HSA?

An excess contribution is taxed at 6% for every year it stays in the account. Remove the excess and its earnings by the due date of your return, including extensions, to avoid the tax. Remember that your employer's contributions count toward the same limit.
